The ultimate goal of having a medical examination performed by Immigration Medical Services is to receive the "Medical and Chest X-Ray Certificate" that's issued by Immigration New Zealand. This medical certificate is necessary for you to get a visa or permit to live, work or study in New Zealand for twelve months or more.
One part of getting the Medical and Chest X-Ray Certificate is filling out the form provided by Immigration New Zealand. This form is is known as "INZ 1007" (or sometimes "INZ1007", "NZIS1007" or "NZIS 1007"). You can download the INZ 1007 form from Immigration New Zealand's website, or you can just drop in to Immigration Medical Services, where we have plenty of them freely available.
The form itself can sometimes be considered "the medical certificate" on its own, but you need to have a medical examination and get the supporting lab tests and x-rays done as part of the medical certificate completion.
